Reducing social isolation

A patient was referred for help with social isolation by the GP. Due to hearing difficulties the patient is unable to hear over the phone so asked that we speak to her granddaughter who cares for her. The carer explained she was having difficulties balancing her caring responsibilities with young children and work. We referred the carer to Carers Link Lancashire and Carers Advocacy to access further support.

The patient is increasingly isolated and feeling vulnerable, so we explained about the befriending support through Advocacy Focus and provided contact details.

We talked about housing options to help the patient feel safer and less isolated and how sheltered accommodation may be more suitable than a care home. We explained about adult social care assessments for help with care and falls alarms and how an advocate can help both the patient and carer to get the right support in place.
